Americans for the Arts has a link to a fairly simple artist contract.

The Graphic Artists Guild has a nice little contract/letter of agreement overview on their site which include a very handy glossary of legal terms.

Matchbook.org has posted a sample contract between an artist and presenting organization. This contract is excerpted from Presenting Performances: A Basic Handbook for the Twenty-first Century by Thomas Wolf. 

Music Contracts 101 offers a template for those in the recording fields. It's fairly involved, but should give you a fairly good idea of what to look for in a contract.
