Final Report Forms

Both the Host Organization and Artist final reports are due thirty (30) days after the completion of the Artists & Communities project.

For Artists & Communities grants, please use the Foundation's eGRANT system to complete your final report at midatlanticarts.egrant.net.  Full instructions were included with your grant award package and are in the eGRANT system. 

Host Organizations:
Log in to the existing account used to submit the project application. The Main Menu of your account will now show a Final Report along with your Artists & Communities application.  Click on the green “Edit” button for the Final Report to begin completing the report.

Artists:
If you do not already have an account on the Mid Atlantic Arts Foundation eGRANT, you will need to register a new account.  Once logged in, select Artists & Communities ARTIST Final Report from the Select a Grant Type drop down menu and click on the Create New Application button.  Click on the green “Edit” button to begin completing your final report.
